1. General Claim Documents
- Completed Claim Form (digital submission).
- Copy of the Insured’s valid identification (KTP/Passport/Birth Certificate/KITAS).
- Proof of airline ticket purchase from tiket.com.
- Bank account number of the Insured or Beneficiary.
2. Specific Claim Documents
For processing a claim, you must provide the following supporting documents:
a. Personal Accident – Death & Permanent Disability (Softcopy)
- Death: Official death certificate from the authorized authorities.
- Missing Insured: Official report of the accident and termination of search from the relevant authorities.
- Permanent Disability: Medical examination report (visum) from a doctor.
- Other documents as required.
b. Flight Delay (Softcopy)
- Proof of delay notification from the airline.
- Other documents as required.
c. Baggage Delay (Softcopy)
- Official letter from the airline stating the duration and reason for the delay.
- Other documents as required.
d. Flight Cancellation (Softcopy)
- Proof that the Insured or a close family member suffered a serious illness, severe injury, or death.
- Copy of court summons if the Insured is called as a witness or jury member.
- Proof of severe damage to the Insured’s primary residence.
- Proof of strikes, riots, or unexpected civil damage beyond the Insured’s control at the travel destination.
- Proof of payment for travel costs and statement of non-refundable travel expenses.
- Other documents as required.
e. Flight Delay (by passenger) (Softcopy)
- Proof that the Insured or a close family member suffered a serious illness, severe injury, or death.
- Copy of court summons if the Insured is called as a witness or jury member.
- Proof of severe damage to the Insured’s primary residence.
- Proof of strikes, riots, or unexpected civil damage beyond the Insured’s control at the travel destination.
- Proof of payment for travel costs and statement of non-refundable travel expenses.
- Other documents as required.
f. Loss/Damage Baggage (Softcopy)
- Statement from the airline confirming that the loss or damage is under the airline’s responsibility.
- Statement from the relevant authority if baggage is lost in a public area.
- Evidence of compensation provided by the airline (if baggage was lost while under airline responsibility).
- A photo showing the damage.
- Proof of Baggage Ownership: If original proof is not available, it can be replaced with:
- Quotation/invoice for the same baggage at the time of the incident; or
- Statement from the Insured containing the purchase date, purchase price, place of purchase, as well as the type and brand of the claimed baggage.
- Other documents as required.
g. Missconnecting Flight (Softcopy)
- Invoice for accommodation issued by the Insured.
- Other documents as required.
